Abstract: Chronic hyperglycemia is a hallmark of the metabolic disorder diabetes mellitus (DM), which is also linked to an increased risk of morbidity and mortality, primarily from cardiovascular and renal diseases. New medications called sodium-glucose cotransporter 2 inhibitors (SGLT2-Is) are used to treat type 2 diabetes and heart failure (HF). SGLT2 mediates protective effects on the cardiovascular and renal systems. This review discusses the state-of-the-art understanding of the biomolecular mechanisms underlying SGLT2-Is’ cardiorenal protective effects, which appear to primarily operate through non-glucose-mediated pathways. Cardiorenal protective mechanisms improve myocardial and coronary endothelial function and slow the progression of chronic kidney disease. In parallel, it is possible to spot changes in biomarkers associated with HF and diabetic kidney disease.
