Abstract: Globally, a major public health issue, chronic obstructive pulmonary disease (COPD) is a common, chronic, and progressive disease. Pharmacological and non-pharmacological methods are used to treat stable COPD. At all stages of the disease, patients with COPD should receive personalized pulmonary rehabilitation, education, and self-management interventions as part of their chronic care. All COPD patients should receive comprehensive care from integrated pulmonary care based on pulmonary centers and a multidisciplinary team. Patients with COPD will live better lives if they are treated holistically, participate in therapy, and work closely with medical professionals. Additionally, it alters the disease’s course, halts progression, and might lessen the frequency of exacerbations. In this article, we discuss the integrated pulmonary care model that has been proposed, the structures required for multidisciplinary treatment, and self-management intervention as crucial components in the care of COPD patients.
