Title: the severity and outcome of febrile neutropenia in adult Filipino patients
Abstract: Aim. The study’s objectives were to outline the patient profile of Filipinos with febrile neutropenia and identify factors that are related to adverse outcomes. Methods. Retrospective analysis of patients with febrile neutropenia from the Philippines who were admitted to the Philippine General Hospital. Patients were categorized based on the presence or absence of severe outcomes and described in terms of their clinical presentation. Regression analysis was then used to find prognostic factors. Results. In 102 patients, 115 febrile episodes were recorded. As significant predictors of poor outcome, regression analysis identified prolonged fever >7 days prior to admission (OR 2.43; 95% CI, 0.77-7.74), pathogen isolation on cultures (OR 2.69; 95% CI, 1.04-6.98), and nadir absolute neutrophil count (ANC) 100 during admission (OR 1.96; 95% CI, 0.75-5.12). Granulocyte colony-stimulating factor (G-CSF) use (OR 0.31; 95% CI, 0.11-0.85) and completeness of antibiotic therapy (OR 0.26; 95% CI, 0.10-0.67) were factors that significantly correlated with a better outcome. Conclusion. While G-CSF use and full antibiotic therapy were linked to better outcomes, prolonged fever >7 days prior to admission, positive pathogen on cultures, and nadir ANC 100 during admission predicted severe outcomes. These prognostic factors may be helpful in determining which patients require more intensive care and monitoring.
