Title: The impact of omalizumab on patients with allergic asthma who have eosinophilic inflammation of the respiratory tract
Abstract: High levels of immunoglobulin E (IgE) and excessive production of pro-inflammatory cytokines, such as interleukins IL-4, IL-13, and IL-5, which are necessary for the production of IgE and the differentiation, maturation, migration, and survival of eosinophils, are two characteristics of bronchial asthma. One of the key cells in allergic inflammation is the eosinophil. Their presence in tissue is associated with tissue damage, remodeling, and the persistence of inflammatory infiltrate. Even when given systemically, some asthmatic patients do not respond to high doses of corticosteroids, despite the fact that these cells are extremely responsive to these medications. In contrast to steroid-sensitive patients, patients with steroid-resistant asthma had higher levels of eosinophils and Th2-type cytokines (IL-4 and IL-5) in their transbronchial biopsies and bronchoalveolar lavages. According to clinical studies, therapy with the anti-IgE monoclonal antibody omalizumab is a very effective treatment option in these circumstances. On the basis of fundamental, clinical, observational studies, and case reports, the paper discusses the effectiveness of omalizumab in lowering the number of eosinophils in peripheral blood and in the airways of asthmatic patients. The role of omalizumab therapy in managing asthma is examined, as well as the mechanisms that control its effects on eosinophils.
