Title: Serological Rh-Del Phenotype Detection Among Rh-Negative Blood Donors at Yangon, Myanmar’s National Blood Center
Abstract: Background. The genetic polymorphism of the RHD and RHCE genes affects the expression of the red cell Rhesus (Rh) antigen and reveals serologically distinct responses to RhD variants like partial D, weak D, and Rh-Del. Rh-Del type can only be identified serologically using an adsorption-elution technique and may be mistaken for Rh-negative. Rh-Del prevalence in Myanmar has not yet been documented. Method. The National Blood Center tested 222 Rh-negative blood donors for weak D and Rh-Del using the indirect antihuman globulin method and the adsorption-elution method, respectively. Rh-Del and Rh-Neg individuals underwent RhCE typing. Results. 75.2% of them (167/222) had Rh-negative blood, 15.8% had Rh-Del blood, and 9% had weak D blood. Out of 202 blood donors, 35 were Rh-Del positive and 167 were true Rh-negative. Of the 35 Rh-Del positives, 94.3% had the Ccee phenotype and 5.7% had the CCee. Eighty two percent (80.2%) of the Rh-negative donors (134/167) had the ccee phenotype. Conclusion. In order to prevent alloimmunization, it was important to avoid transfusing with truly Rhnegative patients since about half of the Rh-Del subjects were repeat donors. It is advised to perform Rh-Del typing on C-antigen positive Rh-negative donors before considering adding them to the Rh-positive pool. The alloimmunization status for transfusion of Rh-Del blood to Rh-negative recipients needs to be clarified through additional research. To improve the selection of antisera for Rh typing and increase the safety of the transfusion services, molecular markers for RhD-negative and D variants should be established in the Myanmar population.
