Title: Patients with Essential Thrombocythemia from Egypt are at Thrombophilic Risk for Factor V Leiden, Prothrombin G20210A, MTHFR, and Calreticulin Mutations.

Abstract: Objectives. One of the myeloproliferative neoplasms, essential thrombocythemia (ET), is characterized by a persistent increase in platelet counts with a propensity for thrombosis and hemorrhage. The purpose of this study is to determine how the thrombotic risk of ET patients relates to their calreticulin, factor V Leiden, prothrombin G20210A, and MTHFR mutations. Methods. This study involved 120 ET patients and 40 people who appeared to be in good health as a control group. Results: When compared to the control group, ETpatients had higher levels of WBCs, PLTcounts, PT, fibrinogen concentration factor V Leiden, and the MTHFR mutation (P 0.05). Additionally, thrombosed ETpatients had higher WBC, PLT, and hematocrit values than nonthrombosed ETpatients (P 0.05). Contrarily, there was no statistically significant difference between patients with cardiovascular risk factors and patients without cardiovascular risk factors (P > 0.05) or between patients with JAK2 V617F positive mutation and the JAK2 negative group. Factor V Leiden, prothrombin gene, and CALR mutations increased the risk of thrombosis in ET patients (odds ratio 5.6, 5.7 and 4.7, respectively). On the other hand, the thrombotic state of those patients is unaffected by the JAk2V 617F and MTHFR mutations. Conclusion. Patients with ET who have CALR mutations, thrombophilic mutations, factor V Leiden and prothrombin gene mutations, as well as a risk of developing leukemic transformation, are at significantly higher risk of developing thrombosis.
