Title: Mapping the Implementation of an Antimicrobial Stewardship Program Led by a Clinical Pharmacist at a Tertiary Care Center in South India

Abstract: Pharmacist roles in antimicrobial stewardship (AMS) programs are still largely unexplored worldwide, including in India. At a tertiary care teaching hospital in Kerala, India, we describe the development and impact of the role of adding clinical pharmacists to a multidisciplinary AMS. Multidisciplinary AMS (February 2016) and antitubercular therapy (ATT) stewardship programmes (June 2017) were established thanks to effective leadership. Clinical pharmacists were added as essential program participants who were in charge of carrying out crucial stewardship interventions. The appropriateness of antimicrobial prescriptions and compliance with AMS/ATT recommendations were monitored by pharmacy-led audit and feedback. Between February 2016 and January 2017, 742 out of 1326 prescriptions for antibiotics were appropriate, and 318 out of 584 recommendations were followed. The appropriateness rate reached 80% (1752/2190) and compliance with the AMS recommendations reached 70% (227/325) by the third year of the AMS. From a baseline of 61% (95/157) in the first year, the appropriateness of ATT prescriptions increased to 72% (62/86, June 2018–February 2019). The percentage of people who followed the ATT recommendations rose from 42% (25/60) to 58% (14/24). In low- and middle-income countries (LMICs), like India, where the lack of infectious disease doctors is a significant barrier to the implementation and sustainability of AMS programs, such a model can be effective in bringing about long-lasting change.
