Title: Is Antimicrobial Photodynamic Therapy Effective in Patients with Chronic Periodontitis in Addition to Scaling and Root Planing? An Organized Review

Abstract: This systematic review sought to determine whether scaling and root planing (SRP) alone was more effective than antimicrobial photodynamic therapy (aPDT) as a primary mode of treatment or as an adjunct to non-surgical treatment in treating chronic periodontitis in terms of clinical attachment level (CAL) gain and probing depth (PD) reduction. The Patient, Intervention, Comparison, and Outcome (PICO) format was used to develop the focused question, and two authors independently searched the Medline, EMBASE, Cochrane Library, Web of Science, Google Scholar, and Scopus databases from January 2008 to December 2016 for pertinent studies. Twenty studies comparing aPDT to placebo, no intervention, or non-surgical treatment in an adult population were randomized clinical trials (RCTs) or quasi-RCTs that were included in this systematic review. We gathered information from the studies about the fundamental study characteristics, photosensitizing agents and wavelengths used in aPDT, frequency of aPDT application, effect of aPDT on clinical parameters, antimicrobial effect of aPDT in chronic periodontitis, effect of immunological parameters after aPDT, and patient-based outcome measures. Even though the studies that were included had a wide range of heterogeneity, they all showed that aPDT has the potential to be a useful adjunct in the treatment of chronic periodontitis. Before aPDT can be suggested as a useful treatment option, long-term, multicenter studies with larger sample sizes are required.
