Title: Ghrelin and obestatin each have their own unique effects that work in tandem with one another.

Abstract: Ghrelin and obestatin are considered to be “sibling proteins” because they are both encoded by the same preproghrelin gene. However, their roles in the body are quite different and intricate. While there is a wealth of research documenting the functions of ghrelin, the roles of obestatin are less well understood and have been the subject of debate. In the beginning, it was thought that ghrelin and obestatin were antagonistic to one another; however, recent studies challenge this dogma. Although in some contexts they have opposing effects, in other contexts they have complementary effects, and the relative importance of many of their functions is still up for debate. In this review, we discuss their functional relationship using three “C” categories, namely complex, complementary, and contradictory, to categorize their interactions with one another. Their roles in digestion, weight regulation, hydration, gastrointestinal motility, inflammation, and insulin secretion are intricately intertwined with one another. Complementary roles in pancreatic beta cells, cardiovascular health, muscle health, neuroprotection, cancer prevention, and the digestive system are played by both of these proteins. In white adipose tissue, thermogenesis, and the regulation of sleep, their functions are mutually exclusive with one another. Overall, the purpose of this review is to contribute to abetter understanding of ghrelin and obestatin, which are both vital gut hormones, by accumulating the myriad functions that ghrelin and obestatin perform under both physiological and pathological conditions.
