Title: Future Prospects for Extracellular Vesicle-Based COVID-19 Therapy: Promises, Challenges, and Current Status
Abstract: Public health and the economy have been adversely affected by the COVID-19 pandemic, which has grown to be a serious concern. Acute respiratory distress syndrome (ARDS) and multiple organ failure (MOF) are its primary targets, and they both increase mortality rates. Therefore, it is crucial to create potentially effective therapeutic approaches for COVID-19 patients. Extracellular vesicles (EVs), which take part in intercellular communication to maintain physiological and pathological processes, are released from a variety of cell types. In systemic hyper-inflammatory states of severe COVID-19, EVs derived from various cellular origins have shown suppressive effects on the cytokine storm, promoting epithelial and endothelial recovery and cell proliferation. Exosomes, the tiniest subclass of EVs, have remarkable properties like cell targeting, nano-carriers for drug delivery, high biocompatibility, safety, and low-immunogenicity, making them a potential cell-free therapeutic candidate against the pathogeneses of different diseases. These characteristics have led to a large number of studies and clinical trials being conducted to evaluate their safety and therapeutic effectiveness against COVID-19. As a result, we have thoroughly discussed in this review the most recent developments regarding the progress and difficulties associated with using EVs as a potential therapeutic agent to treat COVID-19.
