Abstract: Osteoarthritis (OA) is a common and disabling age-related joint disease characterized by osteophyte formation, synovial membrane inflammation, and articular cartilage degeneration. It is also characterized by subchondral bone sclerosis. Currently, most OA medications are palliative in nature and do not slow or stop the progression of the disease. Total joint arthroplasty is still a last resort because there are no disease-modifying OA drugs (DMOADs) on the market right now. Therefore, the creation of effective therapies for the treatment of OA is critical. Exosome-based therapeutic approaches are among the most promising new pharmacotherapeutic options. Proteins, lipids, cytokines, and different RNA subtypes found in exosome cargoes have the potential to control intercellular communication and gene expression in target cells and tissues involved in the development of OA. Exosomes in OA studies are no longer just the traditional, MSC-derived vesicles thanks to recent, extensive research. Exosomes’ origins, structures, and purposes are continuously being discovered and studied. This review explores exosomes’ role in OA pathogenesis and progression, their biosynthesis, extraction, modification, delivery methods, therapeutic potential, and challenges associated with OA treatment. It also describes exosome modification and delivery strategies.
