Abstract: Introduction. In clinical practice, warfarin is a commonly used oral anticoagulant. It requires frequent and routine international normalized ratio (INR) determination to keep the INR within the therapeutic range because it has variable intraindividual and interindividual dose response and a narrow therapeutic index. This study evaluated the anticoagulation control parameters among warfarin patients. Methods. At the hospital run by the University of Gondar, a cross-sectional study was done. The study’s subjects were chosen using a method known as consecutive sampling. The proportion of desired INRs and the percentage of time spent in the therapeutic range were used to assess the anticoagulation control (TTR). To find associated factors with sufficient TTR, logistic regression analysis was used. To declare an association to be significant, a P value of 0.05 was used. Result. The study involved a total of 338 participants, with a mean age of 48.8 (SD 16.4) years. The most frequent reason for warfarin therapy was atrial fibrillation. About one-tenth (13%) of patients had good INR control (TTR 65%), compared to one-third (33%) of study participants who reached the desired INRs of 2.0-3.0. There was no statistically significant correlation between sociodemographic and clinical traits and a successful TTR outcome, according to multivariate logistic regression analysis. Conclusion. Warfarin anticoagulation control among study participants was very low. The authors advise establishing anticoagulation clinics and using a validated warfarin dose titration protocol to address the low anticoagulation level.
