Abstract: During synucleinopathies like Parkinson’s disease (PD), Lewy body dementia (DLB), and multiple system atrophy, different disease-associated forms or strains of -synuclein (SynD) can spread and accumulate in a prion-like manner (MSA). The creation of seed amplification assays (SAAs) that can identify SynD in clinical samples was made possible by this ability to self-propagate. Notably, -synuclein real-time quaking-induced conversion (RT-QuIC) and protein misfolding cyclic amplification (PMCA) assays have developed as ultrasensitive, specific, and generally workable ways to find -SynD in a variety of biospecimens, including brain tissue, CSF, skin, and olfactory mucosa from synucleinopathy patients. However, MSA and familial forms of PD/DLB are still difficult for Syn SAAs to detect, and the assay parameters have poor correlations with a variety of clinical measures. In Syn RT-QuIC assays, end-point dilution analysis enables quantitation of relative amounts of SynD seeding activity, which may moderately correlate with clinical measures and levels of other biomarkers. The modified Spearman-Karber quantification algorithm used with end-point dilutions is thoroughly described here, along with recent developments in -synuclein SAAs for detecting SynD.
