Title: Comparative Analysis of Next-Generation Sequencing in Hematologic Neoplasms from Peripheral Blood and Bone Marrow
Abstract: Objective. It has been clinically interesting to use mutational analysis by next-generation sequencing (NGS) acquired by peripheral blood NGS as a minimally invasive screening tool. In the context of hematolymphoid disease, our study assesses the relationship between NGS findings on peripheral blood and bone marrow. Method. We assessed patients who underwent NGS on bone marrow and peripheral blood within a year of each other for suspected hematologic malignancy. When chemotherapy or a bone marrow transplant took place between two tests, we excluded those cases. Kappa coefficient analysis was used to evaluate the concordance between the NGS results for peripheral blood and bone marrow. Results. There were 163 patients studied in total. 150 patients (92.0%) had bone marrow and peripheral blood NGS concordance, with a kappa coefficient of 0.794 (kappa standard error: 0.054) and a P value for testing kappa of 0.0001. 77/78 cases (98.7%) of myeloid neoplasms had concordant findings, with a kappa coefficient of 0.916. 26/31 cases (83.9%) of lymphoid neoplasms had concordant findings, with a kappa coefficient of 0.599. 47/54 (87.0%) of the non-plastic cases had concordant findings, and the kappa coefficient was 0.743. Conclusion. A trustworthy tool for mutational analysis, peripheral blood NGS offers a less invasive way to screen and keep track of the molecular profile.
