Title: Changing Epidemiology of Polymyxin Use and the Effect of Antimicrobial Stewardship in a South Indian Healthcare Setting

Abstract: Since polymyxins are last-resort medications for treating infections brought on by multidrug-resistant pathogens, antimicrobial stewardship program (ASP) initiatives must be put in place to support their rational prescription in healthcare settings. Our study aims to describe how the implementation of ASP at our institution has changed the epidemiology of polymyxins and patient outcomes. In our 1300 bed tertiary care center’s antimicrobial stewardship program, which was started in February 2016, post-prescriptive audits were used to track polymyxin consumption and assess whether prescriptions were appropriate in terms of the administration of the right loading dose (LD) and maintenance dose as well as the right indication, frequency, drug, and duration of therapy (MD). The number of polymyxin prescriptions dropped from 772 in the pre-implementation period to an average of 417 per year in the post-implementation period, representing a 45% decrease, among the 2442 prescriptions tracked over the entire study period from February 2016 to January 2020. The quarterly prescription appropriateness rates (r = 0.4774, p = 0.02), the appropriate loading dose (r = 0.5228, p = 0.015), and the appropriate duration (r = 0.4361, p = 0.04) significantly positively correlated with the quarterly patient survival rates. In a real-world setting, the successful implementation of antimicrobial stewardship had positive effects on the appropriateness of prescriptions and mortality benefits, according to our study on the epidemiology of polymyxin use.
