Title: A Repeated Cross-Sectional Study in Rural India on the Prescription of Antibiotics for Common Illnesses by Informal Healthcare Providers

Abstract: In rural India, untrained healthcare professionals known as “informal healthcare providers” (IHCPs) are the majority. IHCPs’ prescriptions for antibiotics may contain vital data for managing antibiotic resistance. The purpose of this study is to identify the patterns and seasonal variations in IHCPs’ prescription of antibiotics for common illnesses. In the rural demographic surveillance site in Ujjain, India, a cross-sectional study was repeated over the course of 18 months to account for the various seasons. Twelve IHCPs wrote outpatient prescriptions, which were gathered. 11,336 (74%) of the 15,322 prescriptions examined for 323 distinct complaint combinations contained antibiotics. The findings revealed that 14,620 (95%) of the antibiotics prescribed were broad spectrum, and fluoroquinolones (4771, 31%), extended-spectrum penicillin (4119, 27%), and third-generation cephalosporin (3069, 20%) were the most frequently prescribed drugs. In cases of fever (3569, 87%), upper respiratory tract infections (3273, 81%), oral and dental problems (1126, 88%), and monsoon season (2350, 76%), antibiotics were more frequently prescribed to children (3340, 81%) than to adults (7996, 71%). According to the study’s findings, antibiotics were more frequently prescribed than other medications for common illnesses, the majority of which are broad-spectrum antibiotics. This finding calls for further research, as well as immediate and coordinated efforts to stop IHCPs from prescribing unnecessary antibiotics.
