Title: 2016 Point Prevalence Surveys of Antimicrobial Use Among Children Hospitalized in Six Indian Hospitals

Abstract: India has one of the highest rates of antimicrobial resistance worldwide. Antimicrobial use in inpatient settings is a significant contributor to resistance, but it is poorly understood, especially in pediatric patients in hospitals. As part of the Global Antimicrobial Resistance, Prescribing, and Efficacy in Neonates and Children (GARPEC) project, we conducted four point prevalence surveys in six hospitals between February 2016 and February 2017 to examine the prevalence, indications, and antimicrobial agents used among hospitalized children. Over the course of all survey days, 681 kids were admitted to hospitals, and 419 (61.5%) of them received a prescription for one or more antibiotics (antibacterials, antivirals, antifungals). Third-generation cephalosporins (3GCs) accounted for 38.9% (213/547) of the total antimicrobial prescriptions, while penicillin plus enzyme inhibitor combinations accounted for 14.4% (79/547) of the prescriptions for antibacterial agents. The most frequent reason for prescribing antibiotics was lower respiratory tract infection (149 prescriptions; 24.8%). Despite the fact that penicillin and combinations are advised as first-line treatments for LRTI according to national guidelines, 3GCs were the most frequently prescribed antibacterial drugs (55/149 LRTI prescriptions; 36.9%). In conclusion, there is a chance to reduce the inappropriate use of 3GCs given that 61.5% of hospitalized children were taking at least one antimicrobial agent and using them excessively.
