Title: The Alveolar Epithelium as a Cause of Acute Respiratory Distress Syndrome: New Insights
Abstract: The alveolar epithelium acts as a protective barrier between the body and the outside world. The alveolar epithelium has evolved to withstand and quickly react to a variety of inhaled, injury-inducing stimuli in order to maintain efficient gas exchange. Acute respiratory distress syndrome, which presents clinically as alveolar damage, can result from exuberant, persistent inflammation and loss of alveolar fluid barrier function (ARDS). With a current focus on lung injury caused by viruses, this review discusses recent findings concerning mechanisms of alveolar homeostasis, injury, repair, and regeneration. We also review maladaptive lung responses to alveolar damage that underlie ARDS and pathologic lung remodeling, as well as fresh information on how the alveolar epithelium orchestrates injury-induced lung inflammation.
