Title: Delivery of Ocular Drugs with Specificity: Local Anatomy and Current Methods
Abstract: Research on ocular drug delivery is still largely in the spotlight today. The surface, intravitreal space, subretinal space, and subconjunctival space are the four main routes of administration; each has its own set of special difficulties, restrictions, and benefits. The local anatomy, physical barriers, important cells, and the interface between the anatomy and the drug or drug delivery system all need to be carefully taken into account in each of these methods. Injection into the intravitreal, subretinal, and subconjunctival spaces is direct and targeted but limited due to the many internal clearance mechanisms and potential for eye damage. The topical route is the least invasive but presents a challenge due to the numerous physical barriers that prevent drug penetration into the eye. The development and successful application of a sustained-release system that is well-tolerated, bioactive, biocompatible, and degradable is, in many cases, still in its infancy. Polymeric-based sustained-release drug delivery systems have been identified as a potential remedy for many of these problems. The drugs and biomaterials in question also need special consideration because even minute chemical changes can have profound effects. The four main drug delivery systems are examined in this paper along with their key cells and interface with the body’s anatomy, as well as the most recent advancements and state of the art in each field of study. Finally, this paper reviews the main interactions seen between the commonly used drugs and biomaterials found in ocular drug delivery.
