Title: Male Infertility and the Antioxidant Paradox: Turning a Blind Eye to Inflammation
Abstract: Male infertility is pathophysiologized by a number of interconnected endogenous pathways. According to reports, oxidative stress (OS) is a key mechanism affecting male fertility parameters in about 50% of cases of idiopathic infertility in men. The function of the endogenous antioxidant system is to maintain the seminal redox homeostasis necessary for healthy male recombination. OS occurs when the capacity of endogenous antioxidants is exceeded by a generation of seminal reactive oxygen species (ROS). Thus, the use of antioxidant therapy in the treatment of idiopathic male infertility or subfertility is remarkably relevant. However, use of antioxidant(s) in some cases may be arbitrary or result in overuse and induction of “reductive stress” due to lack of proper OS detection in male infertility. Additionally, OS and inflammation are closely related, and the latter may create a vicious cycle that can disrupt the tissues of the male reproductive system. Male reproductive tissues are disrupted and cellular damage is amplified as a result. As a result, the failure to choose specific treatments that simultaneously target inflammation and OS, two of the primary mechanisms of male infertility, is one of the limitations of antioxidant therapy in treating male infertility. From the perspectives of both inducing reductive stress and ignoring the inflammatory consequences, the current review aims to explain the antioxidant paradox in the treatment of male infertility.
