Abstract: Heart failure’s (HF) precise pathophysiology is still unknown. The Western diet, which is known for its highly sweetened foods, high fat content, fried foods, red meat and processed meat, eggs, and sweet beverages, may lead to inflammation and oxidative dysfunction in the cardiac ultra-structure. It is not well understood how the myocardium’s oxidative function results in physiopathological remodeling and causes HF. In order to maintain the oxidative functions of the heart, antioxidants like polyphenolics and flavonoids, omega-3 fatty acids, and other micronutrients that are abundant in Indo-Mediterranean diets may be protective. Depending on the metabolic needs of the heart, the cardiomyocytes perform physiological functions using glucose and fatty acids. Lipotoxicity has negative effects on the cardiomyocytes in addition to the toxicity brought on by glucose, and these effects are exacerbated by both an absence of endogenous antioxidants and an absence of exogenous antioxidant nutrients in the diet. Individuals may be predisposed to oxidative dysfunction and Ca overloading due to the high-sugar and high-fat-induced production of ceramide, advanced glycation end products (AGE), and triamino-methyl-N-oxide (TMAO). The biological change may begin with the normal remodeling of cardiac cells and progress to biological remodeling brought on by inflammation. The ejection fraction in HF has been found to be preserved by increasing dietary fat content in conjunction with inducible nitric oxide synthase (NOSi) via N-arginine methyl ester. By reducing oxidative damage to lipids, proteins, and DNA in the cell, it is hypothesized that a higher intake of the HEART diet—high exogenous antioxidant restorative treatment—polyphenolics and flavonoids, as well as a reduction in red meat and egg consumption—can improve the heart’s oxidative function and have positive effects in the early stages of HF. Cohort studies and randomized, controlled studies must be conducted to show the HEART diet’s effectiveness in the treatment of HF.
