Abstract: Both hematological and intensivist teams frequently face ethical challenges when admitting patients with hematological malignancies to the intensive care unit (ICU). The choice to transfer to the ICU has significant effects on how patients and their families are cared for at the end of their lives. Additionally, it affects the ICU’s economic and human organizational aspects as well as global health policy. An extensive multidisciplinary discussion has been held in light of recent developments in hematology and critical care medicine, and as a result, both disciplines have unanimously approved guidelines. The primary components included I clarification of the clinical scenarios that could result in a transfer to the intensive care unit (ICU), taking into account the severity criteria of both hematological malignancy and clinical distress, (ii) comprehension of the decision-making process in the context of routine interdisciplinary concertation involving the patient and his family, and (iii) organization of a collegial concertation at the time of the initial decision of transfer to the ICU and throughout. The purpose of this work is to offer recommendations to improve clinical practice, to make daily decision-making easier, and to strengthen collaboration between the various teams involved.
