Title: RBC Alloantibodies in Indian Patients Visiting a Tertiary Care Hospital: Prevalence and Specificity
Abstract: Background. The genetic difference in RBC antigens between donors and recipients leads to red blood cell (RBC) alloimmunization. data on the general patient population’s alloimmunization rate is hard to find, especially in countries with constrained resources. In order to ascertain the prevalence and specificity of RBC alloantibodies among patients admitted in different clinical specialties at a tertiary care hospital in North India, we conducted this study. Methods. 11,235 patients underwent antibody screening using the automated QWALYS 3 platform (Diagast, Loos, France). An 11-cell identification panel was used to identify antibodies (ID-Diapanel, Diamed GmbH, Switzerland). Results. 1.4% (157/11235) of transfused patients developed RBC alloimmunization overall, with anti-E being the most prevalent specificity (36.3%), followed by anti-D (16%), anti-c (6.4%), anti-c + E (6.4%), anti-C + D (5.1%), and anti-K (4.5%). Hematology/oncology patients had an alloimmunization rate of 1.9%, which was higher than that of patients in other specialties, which ranged from 0.7 to 1%. Conclusion. The authors advise pretransfusion antibody screening and the distribution of Rh and Kell matched blood to patients who will likely require frequent transfusions in the future because alloimmunization complicates the transfusion outcomes.
