Title: Outcome of Pediatric vs. Adult Protocols for Adolescents with Acute Lymphoblastic Leukemia

Abstract: Objective. Acute lymphoblastic leukemia (ALL) in adolescents and young adults treated with pediatric protocols had better outcomes than patients of the same age treated with adult protocols, according to some studies, while other studies found no difference in outcomes between the two protocols. In order to compare the effectiveness of our pediatric and adult therapeutic protocols in the management of adolescents with ALL, we conducted this study. Methods and Patients. We looked back at the records of 86 ALL adolescent patients, aged 15 to 18, who regularly attended outpatient clinics between January 2003 and January 2010. 54 of the 86 patients treated by adults followed the BFM protocol, while 32 of the 86 patients treated by pediatricians followed the BFM 90 high risk protocol. In order to achieve complete remission (CR), disease-free survival (DFS), and overall survival, we examined the impact of various treatment protocols (OS). Results. The traits of the two patient groups are remarkably similar. 96% versus 89%, the CR was significantly higher in the pediatrics protocol (P = 0.001). Even though the toxicity profiles in the pediatrics protocol were higher, they were tolerable. Additionally, the pediatrics protocol produced better outcomes for EFS (67% vs. 39%; P = 0.001), DFS (65% vs. 41%; P = 0.000), and OS (67% vs. 45%; P = 0.000). Conclusion. According to the findings of our study, adolescents with acute lymphoblastic leukemia should be treated according to a protocol inspired by intensified pediatrics.
