Title: The Crucial Cardiovascular Risk Factors, Thrombotic Risk Stratification, and Thrombocythemia
Abstract: A rare hematological cancer called essential thrombocythemia has a good overall survival rate but a moderate to high lifetime risk of arterial or venous thrombosis. For patients with essential thrombocythemia, several thrombotic risk scores have been proposed, but only one of them—the IPSET-t scoring system—includes the traditional cardiovascular risk factors as one of the scoring items. Currently, the decision to begin cytoreductive therapies is made in clinical practice based on the presence of cardiovascular risk factors in patients with a diagnosis of ETrarely. In our study, we assessed the influence of common cardiovascular risk factors (hypertension, smoking, diabetes, obesity, and dyslipidemia) on thrombotic risk in patients with ET and compared different risk models to estimate the thrombotic risk of 233 ET patients and the role of specific driver mutations. The impact of cardiovascular risk factors on thrombosis in ET patients should be estimated through prospective studies on a polycentric large cohort of patients. These studies should also assess the potential for starting cytoreductive therapy in patients with cardiovascular risk factors, even if they are categorized into low to moderate risk groups based on other scoring systems.
