Title: Looking for Their Role in Itch in Cutaneous T-Cell Lymphoma with IL-31 and IL-8

Abstract: The itching that is connected to cutaneous T-cell lymphoma (CTCL), such as in Mycosis Fungoides and Sezary syndrome, is frequently severe and poorly responsive to antihistamine therapy. Recent research has emphasized the potential contribution of interleukins to nonhistaminergic itch. We looked into the function of IL-31 and IL-8 in CTCL in terms of the severity of the illness and any accompanying itching. Interleukin- (IL-) 31 and IL-8 levels in the serum of 27 patients with CTCL (17 MF and 10 SS) and 29 healthy individuals (blood donors) were examined, and correlations with disease and itch intensity were determined. Serum levels of IL-31 were higher in SS than MF and in CTCL patients compared to controls. Additionally, the levels of serum IL-31 were higher in patients with advanced disease compared to those with early disease, and they positively correlated with the levels of beta 2-microglobulin, lactate dehydrogenase, and the S’ezary cell count. 67% of CTCL patients experienced itch (MF: 47%; SS: 100%). Serum IL-31 levels were higher in patients with itching than in controls and non-itching patients. There was no correlation between serum IL-8 and itching or disease severity. In CTCL patients, there was a positive correlation between serum IL-8 levels and peripheral blood leukocyte and neutrophil counts. Our study provides a rational target for novel therapeutic strategies by pointing to a role for IL-31 in CTCL-associated itch, particularly in advanced disease and SS. Further research is needed to understand how increased serum IL-8, which has been seen in some patients, can exacerbate itch by attracting neutrophils and encouraging the release of neutrophil proteases.
