Title: A systematic review of the function of surgical resection in Unicentric Castleman’s disease

Abstract: An uncommon benign lymphoproliferative disorder with an unknown etiology is Castleman’s disease. A unicentric form (UCD), which is solitary and localized, and a multicentric form, which is characterized by generalized lymphadenopathy and systemic symptoms, are the two clinical forms of the disease, each with a unique prognosis, course of treatment, and set of symptoms. In order to bolster the evidence supporting the curative potential of surgical treatment for the unicentric type, this article will review recent literature. Materials and techniques In order to find articles addressing the management of the unicentric form of Castleman’s disease, a systematic review of the English-language literature was conducted, and databases (Medline, Pubmed, the Cochrane Database, and grey literature) were searched. Two authors evaluated each article using a structured appraisal tool and stratified it based on the strength of the supporting data. Results: 14 studies were included after applying the inclusion criteria. There were no identified prospective randomized control trials. According to a meta-analysis involving 278 UCD patients, reconstructive surgery is risk-free and ought to be regarded as the gold standard of care. This viewpoint is strengthened by seven retrospective studies. Six studies with disputed findings have used radiotherapy (RT). Conclusions: According to our findings, the most effective treatment for Unicentric Castleman’s Disease of the thoracic cavity is surgical resection. In some patients, radiotherapy can result in a clinical response and a cure.
