Abstract: The primary goal of treating asthma is to manage its symptoms, but despite the availability of prescription medications and treatment regimens, the condition is not sufficiently under control. Due to factors unrelated to the illness, the best control cannot be achieved in cases of difficult-to-treat asthma. Inadequate treatment plans (low therapy adherence and compliance), improper inhalation techniques, inadequate symptom control using the classes of medications available, incomplete response to treatment (non-responders, steroid-resistance), incorrect diagnosis of asthma or comorbidities, and environmental factors are some of the factors that make asthma control worse. Take therapeutic decisions with the patient, evaluate the likelihood of non-compliance, perform thorough diagnostics and start treating coexisting diseases, perform differential diagnosis of conditions that mimic asthma, educate the patient about the inhalation technique and check it, remove unfavorable environmental factors, and modify current treatment are all recommended in order to achieve the best asthma control. Ultra-long-acting beta2-agonists, long-acting muscarine receptor antagonists (LAMA), monoclonal antibodies, and non-pharmacological interventions are some of the new treatment options for asthma patients. Tiotropium bromide is the only LAMA approved for the treatment of asthma. The results of the analyses showed that tiotropium had a high efficacy in terms of bettering lung function parameters and delaying the onset of the first asthma attack. GINA (Global Initiative for Asthma) 2014 recommends it as an add-on therapy at asthma treatment steps 4 and 5. From a medical and financial perspective, it is critical to have the best asthma control possible.
