Title: An update to the systematic review of telemonitoring in COPD
Abstract: In patients with chronic obstructive pulmonary disease (COPD), telemedicine may support individualized care plans, potentially enhancing clinical outcomes. There is currently no conclusive proof that telemedicine benefits these patients. This study’s objective is to provide an update on how well telemedicine can prevent negative clinical outcomes. For articles published between January 2005 and December 2014, we searched the Pubmed database. Only randomized controlled trials with a telemedicine intervention arm and a sole focus on patients with COPD were included. The number of exacerbations, ER visits, COPD hospitalizations, length of stay, and death were evaluated outcomes. Finally, 12 randomized controlled trials were included. The majority of them were of poor quality, had small sample sizes, and a wide range of parameters and technologies. Only three studies reached statistical significance, but the majority of studies found a beneficial impact of telemonitoring on hospitalization for any reason, with risk reductions between 10% and 63%. The same pattern was seen for ER visits and hospital admissions linked to COPD. Telemedicine did not significantly affect how long patients stayed in hospitals, how well they lived, or how many people died. As a result, our study confirms that there is insufficient evidence to draw firm conclusions about the effectiveness of telemedicine in treating COPD. However, the majority of the evidence points to a beneficial impact of telemonitoring on hospital admissions and ER visits. To conclusively prove its efficacy, more studies with adequate sample sizes and background clinical services must be conducted.
