Title: Is the relationship between hypothyroidism and obstructive sleep apnea syndrome causal or just coincidental?

Abstract: Obstructive sleep apnea-hypopnea syndrome (OSAHS) is more common in men (4–7%) than in women (2–5%). It is a significant health issue due to its harmful effects, which include cardiovascular complications that increase morbidity and mortality and traffic accidents. Patients with hypothyroidism are more likely to exhibit this phenotype than obese people, who are a major risk factor for OSAHS. Despite the fact that hypothyroidism appears to be a risk factor for OSAHS, some authors claim there is no clinically meaningful association. The argument is based in part on the fact that OSAHS patients have a higher prevalence of hypothyroidism, but the epidemiological evidence is sparse and occasionally contradictory. Furthermore, little is known about sub-clinical hypothyroidism in OSAHS patients. Even though the frequency of overt and sub-clinical hypothyroidism in OSAHS patients is comparable to that of the general population, thyroid hormone replacement therapy may help with sleep disordered breathing, so screening for it seems beneficial. Unfortunately, this positive result was only observed in a small number of studies involving a small number of hypothyroidism patients. Although there are no comprehensive multicenter studies on the subject and no international recommendations, we believe that TSH screening may be helpful for the vast majority of OSAHS patients.
