Title: A systematic review of acute eosinophilic pneumonia linked to non-cigarette smoking products

Abstract: An acute respiratory illness with bilateral chest infiltrates and evidence of pulmonary eosinophilia is known as acute eosinophilic pneumonia (AEP). The main risk factor is cigarette smoking, but there have also been reports of drug use and other inhalational exposures. Here, the relationship between AEP and non-cig smoking devices is examined. The terms “smoking,” “vaping,” “e-cigarette,” “waterpipe,” and “marijuana,” as well as their common synonyms, were searched in the PubMed database. The same database was also used to look for eosinophilic lung diseases. Using the modified Philit criteria, all cases of AEP were found to be related to the use of marijuana, waterpipes, e-cigarettes, or heat-not-burn cigarettes. Cases involving the use of illegal drugs were excluded. Twelve cases with a median age of 20 were included (15–60). Men made up 75% of the study’s participants. Five people reported using marijuana, two used waterpipes, two used heat-not-burn cigarettes, two used e-cigarettes, and one used synthetic cannabinoids as exposures. In 50% of patients, a recent change in smoking behavior was disclosed. Dyspnea (91.6%), cough (66.6%), fever (66.6%), and chest pain (25%) were the symptoms that were present. Only 16.6% of patients had peripheral eosinophilia at presentation, while 90% of patients had leukocytosis. In bronchoalveolar lavage, the median eosinophil percentage was 67.5%. (0 to 78). The lu biopsy was carried out on two patients. On chest imaging, all patients had bilateral involvement. Ten patients (83.3%) were treated in an intensive care unit, and five patients (41.6%) required invasive mechanical ventilation. With no relapses reported, all patients responded to corticosteroid therapy. Acute eosinophilic pneumonia has been associated with smoking, including waterpipes, e-cigarettes, heat-not-burn cigarettes, and marijuana, and can present similarly and progress clinically.
