Abstract: Mechanical ventilation (MV), both invasive and non-invasive, is still the most important technique for life support. But it also comes with a lot of risks. Historically, MV concepts did place more of an emphasis on enhancing arterial blood gas results than on guarding against negative effects of positive pressure ventilation. Since then, a number of studies have looked into this issue, giving rise to the protective MV concept. It’s still up for debate what the ideal balance is between ensuring optimal oxygenation and minimizing ventilator-induced lung injury (VILI). These factors are particularly significant when treating patients with adult respiratory distress syndrome (ARDS), where it is crucial to keep the negative effects of MV to a minimum. The protective ventilation idea and its clinical ramifications are examined in this essay.
