Abstract: Numerous biomarkers have been employed over time to help with disease screening, diagnosis, prognosis, and therapeutic response. Recent progress in the identification and characterization of proteoforms—a distinct functional pool of proteins—has given protein biomarkers a stronger role in the early diagnosis and prognosis of disease. Proteoforms are all the various protein molecules that arise from a single gene as a result of genetic variations, alternative RNA splicing, and post-translational alterations. To understand the intricate molecular mechanisms that control how a disease develops, it is crucial to keep track of the structural alterations of each proteoform of a specific protein. Therefore, clinical proteomics has the potential to shed light on the pathophysiology, course, and prevention of disease. Nevertheless, to increase the accuracy and clinical applicability of proteomics in preventive medicine, more cutting-edge diagnostic techniques are required. The use of immunoaffinity capillary electrophoresis (IACE) as a powerful diagnostic tool to isolate, separate, detect, and characterize proteoform biomarkers obtained from liquid biopsy is discussed in this manuscript. Affinity capture-separation technology known as IACE is able to isolate, concentrate, and analyze a variety of biomarkers found in biological fluids. Target analytes are isolated and concentrated by binding to one or more immobilized biorecognition affinity ligands, and separation and analysis are carried out using high-resolution capillary electrophoresis (CE) connected to one or more detectors. IACE has the potential to produce prompt outcomes with noteworthy accuracy, resulting in dependability and reproducibility in disease diagnosis and monitoring. IACE also has the ability to quantify ancillary and complementary protein biomarkers in order to track the efficacy of therapeutic agents. The application of proteoform biomarker detection and analysis may significantly increase our ability to identify medical conditions early and intervene in ways that improve health outcomes for individuals and populations thanks to developments in telemedicine and artificial intelligence.
